Admission formalities

The admission and discharge formalities are carried out at the secretariat of the hospitalization department (secrétariat du service d’hospitalisation), located in the reception hall, on the ground floor of the establishment.

 

Pre-admission

During an appointment with your specialist, you will be provided with information on the planned medical or surgical procedure, on possible complications and on the fees and possible overruns. A Patient Information Booklet ( Livret d’Information Patient )  including a Patient Admission Booklet (Livret d’Entrée Patient) will also be given to you during this consultation with your practitioner.

Before the pre-anesthetic consultation, we ask you to go to the Admissions Office (Bureau des Admissions), in order that your administrative file can be prepared.

The pre-anesthetic consultation is mandatory.  Anesthesia is a set of techniques allowing the performance of a surgical or medical act (endoscopy, radiology, etc.) by eliminating or reducing pain.

The pre-anesthetic consultation must be carried out before any general or locoregional anesthesia several days before the operation; do not forget to bring with you any recent laboratory results, your medical treatment and your Patient Entry Booklet (Livret d’Entrée Patient) including the pre-anesthetic questionnaire, all duly completed and signed.

For the pre-anesthetic consultation, as well as for the pre-admission formalities, you must bring the following documents:

  • Your valid identity card, passport, driving licence issued by an EU state, or residence card ("carte-de séjour")

  • Your valid "carte Vitale" or other health insurance card

  • Your complementary health insurance card ("carte mutuelle") or agreement if you have such a complementart insurance.

  • Your current prescriptions and medications if you are on treatment

  • Results of any additional examinations such as  biological analyzes, X-rays, CT, cardiological examination (ECG)...

  • Your blood group card, if you have one

 

And depending on your circumstannces :

  • Proof of any 100% state payment

  • Free healthcare booklet ("carnet de soins gratuit") if you are a beneficiary of Article115 (veterans and victims of war)

  • Triptych or accident declaration if your hospitalization is due to a work-related accident

  • Certificate of Complementary Health Solidarity (CSS)

 

In addition to the documents listed above:

If the patient is a MINOR, it is mandatory to present the following documents:

  • A valid Identity card, passport, driving license issued by an EU state, or a valid residence card of the person or persons having parental authority

  • The livret de famille

  • The health and vaccination records

  • Surgical and anesthetic informed consent forms giving authorization to anesthetize, operate, and perform care, signed by the person or persons having parental authority (see the Patient Admission Booklet)

  • In the event of hospitalization for voluntary termination of pregnancy ("IVG") in the absence of parental consent, the minor must be accompanied by an adult of her choice (Circular of September 26, 2001).  Bring your family planning certificate with you.

  • All minors must be accompanied during their hospitalization by the father, mother, legal guardian or by an authorized person. An accompanying person's identity document must be presented to the ward nurse, a copy will be kept in the patient file.

For a PATIENT UNDER GUARDIANSHIP: bring the decision of placement under guardianship and the guardian's identity document.

 

Admission

The admission and discharge formalities are carried out by the secretariat of the hospitalization department (secrétariat du service d’hospitalisation):

BEAUREGARD 1 :

  • Maternity : Level 2 from 8:30 a.m. to 1:00 p.m. and from 1:30 p.m. to 6:30 p.m. from Monday to Saturday

  • Gynecological surgery / Cosmetic surgery: Level 3 from 8:00 a.m. to 12:30 p.m. and from 1:30 p.m. to 4:00 p.m. from Monday to Friday

BEAUREGARD 2 : from Monday to Friday

  • Digestive surgery and Resuscitation: Level 1 from 9:00 a.m. to 12:30 p.m. and from 1:30 p.m. to 5:00 p.m.

  • Orthopedic surgery and Continuous medical surveillance: Level 2 from 8:30 a.m. to 12:30 p.m. and from 1:00 p.m. to 4:00 p.m.

  • Cosmetic surgery - ORL - Vascular surgery - Cardiology - Cardiological intensive care: Level 3 from 9h00 to 12h30 and from 13h30 to 17h00

BEAUREGARD 3 :

  • Endoscopy: Ground floor from 7:00 a.m. to 12:30 p.m. and from 1:00 p.m. to 2:30 p.m. Monday to Friday

  • Medicine: Ground floor from 7:00 a.m. to 12:30 p.m. and from 1:00 p.m. to 2:30 p.m. from Monday to Friday and until 4:00 p.m. on Tuesday and Thursday

  • Chemotherapy: Level 3 from 8 a.m. to 4 p.m. Monday to Thursday and 8 a.m. to 1:30 p.m. on Friday

  • Outpatient: Level 4 from 7:00 a.m. to 12:30 p.m. and from 1:00 p.m. to 2:30 p.m. Monday to Friday

 
Outside the opening hours of the departmental secretariats, admissions are made with the emergency secretariat: Rez-de-Chaussée Beauregard 2.

 

For your admission, you must bring the documents mentioned above.

 

A deposit will be requested upon admission and will be kept until your treatment has been fully paid (the hospital reimbursed by the Social Security and your complementary health insurance). 

A flat-rate contribution of 24 €, for the benefit of the Assurance Maladie, will be payable upon admission (n ° 2018-1257 of December 27, 2018). 

A flat-rate increase of € 15 will be applied to any invoice for which you are responsible and for which payment remains outstanding more than one month after the date of your discharge. 

Due to the increasing number of unpaid invoices, no exceptions can be made.

 


If your hospitalization is urgent, the admission formalities are reduced to a minimum, and the file will be completed later by the patient themself at their bedside, or by a member of the family at the admissions office. Requesting this information may seem inappropriate at such times, but we thank you for your understanding. Your identification is important for both administrative and medical reasons. Our hospital works in close collaboration with the 24-hour on-call service organized by the local family/general practitioners. Your family or general practitioner will be informed, if you wish, of the progress of your treatment. At your request, he may consult your medical file in accordance with the rules of ethics.

 

Special cases:

  • We are counting on your assistance to provide us with the necessary supporting documents, in particular those concerning your complementary health insurance ("mutuelle").  An their absence a provision corresponding to the estimated amount of your hospital fees may be required.
  • If you do not have any social security cover or if your cover has not been accepted, you will have to pay the full cost of hospitalization. You will then be asked for a deposit upon admission, the amount of which will vary depending on the type of intervention and any costs that may be payable by you (daily rate, television, telephone, private room, etc.). This check will be returned to you by mail after full payment of your invoice.
  • If you are not covered by French Social Security, an estimate will be provided to you for signature and a deposit will be required.

  • Do not forget to inform your employer of your absence, by sending him the "bulletin de situation" which will be given to you by the discharge office or which will be given to you at your request in the event of outpatient surgery.
